COVID-19 digital vaccination certificate

The COVID-19 digital vaccination certificate can be issued by using the My Number card. (The paper version vaccination certificate is also available.)

　・The contents are the same as the certificate in paper.
　・Counterfeit can be prevented by the electronic signature.

What you need for the application
　Please make sure that the necessary items differ between the “domestic version” and the “international version.”

○Necessary items for applying for the domestic version
　1.My Number card
　　Please apply from here if you do not have it.
　　※It will take around one month and half after applying to issue.
　2.PIN
　　The 4-digit PIN registered in the My Number card will be necessary.
     Please contact the Toyohashi My Number Call Center (0532-43-5505) if you forgot the PIN.
  3.Devices such as smartphones which can read the My Number Card.

     (Devices which support NFC Type B) (iOS 13.7 and above, Android 8.0 and above)
  4.App
　Download the app from the following.
　　App Store (iOS)　
　　Google Play (Android)　
　　Please check the web site of the Digital Agency for details.

　　※Start the dedicated app and apply according to the “app operation manual.”

○Necessary items for applying for the international version
　In addition to the ① to ④ listed above, ⑤ listed below is necessary.
  5.Passport (which is not expired)
　※The international version can also be used for domestic use.


Those who cannot use the digital application
Those who apply to any of the followings cannot use the digital application.

　・Who do not have a My Number card.
　・Who do not have the environment (such as the device and version) to use the app.
　・Who has the maiden name, another surname, or another name listed on the card.
　・Who will travel without a passport.

The paper version vaccination certificate is also available. The paper version vaccination certificate will also have a 2D barcode.
