About the Home Medical Treatment Certificate  (新型コロナウイルス感染症自宅療養証明書について)

A document that proves you recovered from a COVID-19 infection at your home following a doctor’s diagnosis (only available in Japanese)

※Only individuals who have a COVID-19 Infection Notification (Hassei-todoke) submitted to the Hokenjo Health Center by the doctor who examined them will get a Home Recovery Certificate. From September 26, 2022 to May 7, 2023, only the following individuals (listed in chart below) will have notifications submitted on their behalf and be eligible for Home Recovery Certificates. From May 8, the Hassei-todoke will not be submitted at all, so the Hokenjo will no longer issue Home Recovery Certificates.

【Please Note】
・For proof of recovery at a hotel or similar lodging, please contact Aichi Prefecture (TEL 052-961-2111). Ask for a“Shingata Coronavirus Kansenshou no Shukuhaku Ryouyou Shisetsu ni Okeru Zaisho Shoumei” (essentially, proof of recovery from COVID-19 at an overnight recovery facility).
・For proof of a period of hospitalization, please ask the facility in which you were hospitalized. 

■Information on Certificate
The date you began your home recovery (the date you were diagnosed) will be written on the Home Recovery Certificate issued by Toyohashi (the date you were diagnosed is not the same as the day you became infected or the day symptoms appeared). Depending on the date they are diagnosed by a doctor, individuals who complete home recovery within the standard time period (home recovery period of no more than 10 days) will have the date they completed home recovery omitted from their certificate.

(Example) You develop symptoms on April 30 and decide to isolate at home.  You are then examined at a medical facility on May 3, and on May 4, you receive confirmation that you tested positive for COVID-19. If you are notified by the Toyohashi Hokenjo Health Center on May 7 that you have completed your home recovery period, that period is considered May 4 – May 7, and the final day will be omitted from your certificate.

【Applying by Phone】
Please contact the Kansenshou Taisaku-shitsu (Infectious Disease Control Room) if you need a Home Recovery Certificate.

You will be asked for your address (beginning with the postal code) when applying by phone

【Applying in Person】
Please come to the Kansenshou Taisaku-shitsu on the 2nd floor of the Toyohashi Hokenjo Health Center (Hoippu) 
Open weekdays, 8:30 AM to 5:15 PM* *(excludes New Year’s, national holidays)
Please have your official ID (one of a Zairyuu card, My Number Card, Driver’s License, etc.) ready when you approach the reception window.

《Notice》
●The person for whom the certificate is for should come if applying in person (if they are not of age, their guardian can apply on their behalf)
●We cannot complete any insurance company forms, etc.
●One copy of the certificate will be issued per application. Please make additional copies on your own if they are necessary.
●If you use a home test kit or are examined but not tested (presumed positive), the health center will not be informed and you will not receive a certificate.


《Time until certificate is issued》
If you apply by phone, your certificate will be mailed to you once all of your information is confirmed.
If you apply in person, we will give you your certificate directly once all of your information is confirmed.
