COVID-19 Self-Isolation Certificate (Jitaku Ryouyou Shoumeisho)
The COVID-19 Self-Isolation Certificate (Jitaku Ryouyou Shoumeisho) is a document that proves you isolated at home (or in a designated facility) while recovering from a COVID-19 infection. It is only available in Japanese.
※You are only eligible for a Self-Isolation Certificate if you were diagnosed with COVID-19 and your doctor submitted a COVID-19 Infection Notice (Hassei Todoke) before May 7, 2023.
If your doctor did not issue a COVID-19 Infection Notice, you are ineligible to receive a Self-Isolation Certificate.
※If you will not have a designated period of home isolation (because you are diagnosed with COVID-19 after a period time equivalent to the recovery guidelines has passed, etc.) we cannot issue a Self-Isolation Certificate.
※We cannot issue certificates or complete forms for private insurance companies.
On September 1, 2022, in response to a request from the Financial Services Agency (FSA), life insurance companies were asked to consider establishing an administrative structure that would not require Self-Isolation Certificates (Jitaku Ryouyou Shoumeisho) from the insured individual when paying out benefits. The government has also asked that companies and schools not ask for the certificates.
About the Certificate
The COVID-19 Self-Isolation Certificate (Jitaku Ryouyou Shoumeisho) is a certificate that proves you quarantined/isolated yourself from others from the day of your diagnosis until the day you were deemed to be finished with your recovery.
Furthermore, based on a notice from the Ministry of Health, Labour and Welfare (MHLW), if it is confirmed that you isolated for 10 or fewer days, the end date of your isolation will be omitted from your certificate.(※)。If this is the case, only the day you started isolating will be included on your certificate. It will not reflect the period of time you isolated.
(※)We have received many inquires about the omission of the isolation period. However, the MHLW has already discussed this situation with the Japan Medical Association, the FSA, the Life Insurance Association of Japan, as well as the General Insurance Association of Japan. For further handling of your situation, please contact the appropriate insurance company.
【Warning】
・For proof of time spent isolating/recuperating at a hotel or other facility, please contact Aichi Prefecture (Tel: 052-961-2111 (ask about proof of stay (Zaisho Shoumei) at a hotel/facility for COVID-19 infection recovery (Shingata Coronavirus Kansenshou no Shukuhaku Ryouyou Shisetu))
・For proof of time spent hospitalized, please contact the medical facility where you were hospitalized.
・Time you spent recovering before you were diagnosed and time spent isolating based on your own judgement of your condition will not be certified.
(Example) Symptoms appear on April 30 and you decide to isolate at home, then you go to the doctor on May 3 and are confirmed positive on May 4. You receive communications from the Toyohashi City Health Center on May 7 stating you can end your isolation. Even if you decide to isolate for two more days until the end of December 10, a final date will be omitted from your certificate.
COVID-19 Self-Isolation Certificate Application
If you need a Self-Isolation Certificate issued because you can’t access My HER-SYS, etc., please apply for one.
※If you were diagnosed with COVID-19 before May 7, 2023, your doctor submitted a COVID-19 Infection Notice, and you received a call from the health center (Hokenjo) regarding the observation of your health, a certificate will be mailed to you at the end of your recovery period and you do not need to apply.

We will ask for your HER-SYS ID, mailing address (beginning with the postal code), etc. for your application.
【Applying in Person】
Please come to the Kansenshou Taisaku-shitsu on the second floor of the Toyohashi-shi Hokenjo Health Center (Hoippu)(Weekdays from 8:30 AM to 5:15 PM)
Please have your HER-SYS ID and an official form of ID prepared when coming in person.
Notice
●The person for whom the certificate is intended (or their guardian if they are a minor) should be the person to apply.
●We can’t provide proof or complete forms for insurance companies.
●One certificate will be issued per application. Please make copies on your own if you need multiple copies.
●You cannot get a certificate by using a COVID-19 home test kit, as a diagnosis at a medical facility and contact with the health center is required.
●If you are examined at a medical facility but are not tested and instead “presumed positive,” the health center will not be informed of your case and you cannot get a certificate.
Issuance
If you applied over the phone, your certificate will be sent by mail after your information has been confirmed.
If you apply in person, you will be handed your certificate after your information has been confirmed.
