Application of the COVID-19 paper version vaccination certificate

 

新型コロナウイルス感染症予防接種証明書の紙での発行について

 

We will accept the COVID-19 vaccination certificate which officially certifies the receiving of the COVID-19 vaccination.

 

Eligible people

Those who got the COVID-19 vaccination using the voucher (or health form with the voucher attached) issued by Toyohashi City.

 

Application destination

In principle, the application must be done by mail.

Please note that the certification cannot be issued on the same day as the application.

 

Required documents

Please make sure that the necessary items differ between the “domestic version” and the “international version.”

 

 ○Necessary items for the domestic version of the certificate.

 

(1)Application form

(2)Copy of the documents which can confirm the vaccination history.

COVID-19 vaccination certificate or the vaccination record.

 

(3)Copy of ID

Such as driver’s license, health insurance card, My Number card. ※Passport can also be used if the address is listed.

 

(4)Reply envelope (which has the reply address and required fee of postage stamps.)

There are many cases of the shortage of postage stamps. Standard-size mail is \84 for less than 25g, and \94 for 26g to 50g.

Please make sure to put the right amount of stamps when applying by mail.

 

 ○Necessary items for the international version of the certificate.

In addition to (1) to (4) listed above, (5) listed below is necessary.

 

(5)Copy of the passport which is not expired.

 

The following documents may be necessary in some cases.

 

If the maiden name, another surname, or another name listed on the passport.

Copy of the ID (such as driver’s license) which has the maiden name, another surname, or another name listed.

 

Application by an agent.

An authorization letter written by the delegator.

 ※It is necessary also for members of the same household.

 ※A copy of the agent’s ID is necessary.

 

Notes

 

Please apply to the municipality where you were registered when you got the vaccination.

Example) If you were registered in Toyohashi when you got the first dose and in another municipality when you got the second dose, the certificate of the first dose will be issued in Toyohashi and the second dose will be issued in the other municipality.

 

The name which will be listed on the certificate must be the same as the passport. If you wish to have the alias name listed together, please correct the passport first.

 

It take around a weeks to issue the certificate. It may take time if there are documents which are incomplete. If it is urgently needed do to the scheduled travel date, please contact the Toyohashi COVID-19 Vaccination Call Center.

One certificate will be issued for each application.

The certificate is a vertical A4 sized paper.

Please check the website of the Ministry of Foreign Affairs for information on which countries and regions the certification can be used.
